Your name. Slide 1: Briefly describe the problem indicating the “burden of disease” includingmorbidity and mortality and changes that have occurred in recent years. Justifywhy you have chosen this specific disease using Healthy People 2020/30 and/oranother public health initiative if HP2020/30 isn’t relevant to your issue. Slide 2: Expand on the burden of disease locally compared to state and/ornational trends using data from County Health Rankings. Justify why you havechosen a different local data set if HP2020/30 isn’t relevant to your issue. Slide 3: Briefly describe the course of the disease or condition using the conceptsof incidence, prevalence and case-fatality. If these concepts are not relevant toyour subject explain what they mean and why they are not relevant. Slide 4: Identify at one or two at-risk groups (those with an increased probabilityof developing and/or experiencing bad outcomes), and explain why these groupsare at-risk of developing your health issue (be sure to use course concepts). Slide 5: Identify 2 options for potential interventions to address the problem. Foreach of the options identify the primary focus in terms of When (Primary,secondary or tertiary prevention), Who (individual, at-risk group, population) andHow (education, motivation, obligation, incentive). Number each of the potentialinterventions. Identify each option then bullet or create a table giving youranswers to When, Who and How for each option. From these potentialinterventions select 1 that you identify as most effective.
